The owner runs the practice and the billing. Admins are for office staff — they see every case and manage members and assignments, but never billing. Monitors carry cases. Admins and office staff are free, however many you add.
Invite by email from My Team. Each person accepts and creates their own credentials, so nobody shares a password and every action on a case is attributable to the person who took it.
Set the assigned monitor when you open a case, or change it later from Edit Case. Only owners and admins can assign, and a case can sit unassigned while you decide who takes it.

No — a monitor sees only the cases assigned to them. That boundary is enforced in the database by the same row-level security that separates one practice from another, so it is not a matter of the interface hiding rows.
An admin sees every case in the practice, invites members, and assigns cases. What an admin cannot do is manage billing, or act on the owner or on another admin. Admins are free.
It stays on the case. Removing someone ends their access immediately, including to the cases assigned to them, but the visits they documented and the reports they wrote remain part of the record — as they must, since those are evidence.
